Asthma

In Australia, almost 2 million people suffer from asthma. In every 4 primary school children 1 suffers from it, 1 in 7 teenagers and 1 in 10 adults are affected by asthma. There has been a rise of Asthma and other topic allergic disorders in the world. Australia has the third highest prevalence of current wheeze in 13-14 year olds.

Our experienced GPs know the importance of diagnosing and managing asthma. They use peak flow meters and refer patients for spirometry. Our GPs perform regular reviews and provide asthma management plans, preventive and reliever medications as suitable. For those who have severe disease they refer them to respiratory and allergy specialists.

Heart Disease

Heart Disease is the leading cause of death in australia, killing 51 Australians every day Heart Foundation. Managing your lifestyle through minimising smoking and alcohol, staying active and improving fitness, and managing weight and diet ensures you minimise your risk of heart disease. Maintaining regular contact with a practitioner will help them educate and diagnose risk conditions, to ensure you stay healthy long term.

COPD

COPD stands for Chronic Obstructive Pulmonary Disease, which describes a range of lung diseases including Smphysema, Chronic Bronchitis and Chronic Asthma amongst others. A spirometry test will help to confirm a diagnosis, and while there is currently no cure, there are a variety of measures that can be used to slow the disease progression and minimize symptoms. These include excercise programs, medicines (including inhalers) and ensuring keeping up-to date with influenza and other respiratory disease vaccinations and medication.To ensure quality of life, our practice, nurse and doctors will maintain regular contact and support you in an ongoing basis.

GP Management Plans and Team Care

Our Doctors provide GP Management Plans and Team Care Arrangement for patients with chronic medical conditions. These conditions include cancer, asthma, diabetes, cardiovascular disease, stroke and musculoskeletal conditions. These plans form a framework for the doctor and patient to agree on goals and actions through diagnosis and discussion, enabling management and symptom relief on an ongoing basis. As patients action the plan, this can in turn be supported and updated by the doctor on an ongoing basis to ensure the choice remains with the patient.

For eligible patients, our GPs have access to allied services such as Physiotherapist, Exercise Physiologist, Podiatrist or Dietitian, among others, which can help bring down the costs of treatment. Under this Team Care Arrangement, the patient will be eligible for up to five subsided health services each calendar year. In these cases, the management plans help facilitate communication between GPs, specialists and Allied Health professionals to enhance patient care. This structured approach in turn helps the doctors, allied health practitioners and specialists plan and coordinate their care requiring ongoing support from a multidisciplinary team.
